Poznámka: Note: | modernizovaný z Mi-17 0842, vrtuľník slúžil v KFOR 2009, - slúžil v EUFOR (ALTHEA) Vo štvrtok 9. 10. 2003 popoludní došlo k havárii vojenského vrtuľníka Mi-17 0842, ktorý patril 3. leteckej základni Prešov. Vrtuľník sa zrútil do zalesneného porastu v katastri obce Pušovce, približne 12 km severovýchodne od mesta Prešov. Pri páde stroja zahynuli všetci štyria členovia posádky. Všetci muži na palube (kapitán vrtuľníka – kpt. Ing. Pavol Gromovský, pilot inštruktor – pplk. Ing. Jozef Sagan, palubný technik – rtn. Slavomír Seman a pilot vo výcviku – kpt. Ing. Peter Nemčík) boli príslušníci ozbrojených síl Slovenskej republiky. Na miesto havárie bola vyslaná vyšetrovacia komisia pod vedením zástupcu náčelníka Generálného štábu OS SR generálmajora Ing. Jozefa Blizmana. V rámci vyšetrovania sa vyhodnotila technická a letecká dokumentácia vrtuľníka. Vrak stroja bol odborne demontovaný a technické expertízy prebiehajú priamo na mieste. Ďalšie expertízy budú prebiehať po prevezení vraku na 3. leteckú základňu. „Na príčine bola technická závada, kde v mraku nastala situácia, ktorú si pilot nacvičil. Zlyhali prístroje a osádka nerozpoznala, kde došlo k zlyhaniu prístrojov a defacto sa riadila ďalej podľa chybných údajov z prístrojov. Vrtuľník bol vtedy uvedený do priestorovej polohy, ktorá sa už nedala zvládnuť. Situácia bola o to horšia, že oblačnosť bola skoro až po zem. Keď vyleteli z oblaku, už nemali čas zrovnať vrtuľník, pretože videli, že sú v zlej polohe podľa prirodzeného horizontu. Keď vyleteli z oblaku, videli horizont niekde pred sebou, ale už to nestihli, boli veľmi nízko.
